Position Overview:
As a Palliative Care RN, you will play a vital role in providing compassionate care to patients in hospice settings, whether in private homes or healthcare facilities. Your expertise will be essential in ensuring that patients receive comprehensive support tailored to their individual needs.

Work Schedule:
Options available include:
- Monday to Friday, daytime shifts with no night duties
- On-call rotations with a 7 on 7 off schedule
- Weekend on-call opportunities

Benefits:
We offer a competitive salary range of $78,000 - $86,000 per year, along with a comprehensive benefits package that includes:
- Medical, Dental, and Vision coverage
- Three weeks of vacation plus six holidays
- Life Insurance
- Tuition Reimbursement of up to $2,000 annually
- Reimbursement for obtaining a Hospice Certification
- An annual bonus of $1,000 for maintaining your hospice certification
- Scrubs provided for your convenience

Key Responsibilities:
  1. Care Coordination: As a case manager, you will organize and oversee the holistic care provided to hospice patients, ensuring all their medical, emotional, and practical needs are met.
  2. Patient Assessments: Regularly assess patients' conditions and needs to develop and adjust personalized care plans aimed at enhancing comfort and quality of life.
  3. Family Support: Provide education and emotional support to patients and their families, addressing concerns and guiding them through the hospice process.
  4. Team Collaboration: Work closely with a multidisciplinary team, including physicians, social workers, and counselors, to promote effective communication and coordinated care.

Qualifications:
- Active RN license in the state of practice
- Previous experience in hospice care is highly preferred
