Key Responsibilities:
  • Conduct thorough reviews of patient medication and treatment plans for managed care populations, ensuring the appropriateness of care levels.
  • Facilitate interdisciplinary meetings focused on the continuum of clinical care and serve as a resource for discharge planning and community support.
  • Act as an advocate for patients by preparing case documentation, care plans, and discharge strategies aligned with managed care requirements.
  • Ensure that our short-term skilled nursing patients are medically stable and have safe discharge plans to foster community success and minimize readmissions.

Qualifications:
  • Valid state LPN license preferred; PTA and OTA candidates are encouraged to apply.
  • Understanding of Managed Care principles and a commitment to the nursing needs of the elderly and chronically ill.
  • Experience in skilled nursing facilities, clinical reimbursement, concurrent review, and/or utilization management is advantageous.
  • Familiarity with Medicare guidelines, InterQual, and/or McKesson is beneficial.
  • Strong communication and interpersonal skills.
  • Proficiency in Microsoft Office 365.
  • Able to work effectively within a multidisciplinary team.
