Job Title: Intensive Care Manager

Job Summary:

An Intensive Care Manager is responsible for providing clinical leadership and coordination of recovery support and behavioral health services for clients with severe mental illness. The ICM works with clients, providers, and other stakeholders to develop and implement individualized recovery plans, ensuring a smooth transition into or out of the community.

Key Responsibilities:

  • Assumes responsibility for a designated client case load;
  • Travels statewide to assess and evaluate potential clients;
  • Completes all MHW/DSS paperwork as required;
  • Works as part of a team providing clinical expertise and knowledge to other team members;
  • Provides clinical supervision for LCSW candidates working towards licensure;
  • Conducts all recovery plan reviews, service authorizations, and care coordination for assigned clients;
  • Maintains an understanding of behavioral health benefits and remains current on covered benefits, limitations, exclusions, and policies and procedures;
  • Supervises collection of information regarding the delivery and outcomes of services to clients, and uses that information to recommend modifications to plan policies and procedures;
  • Provides clients, providers, and other stakeholders with accurate and timely information concerning waiver benefits and coverage;
  • Works with clients and providers to customize services to best meet client needs within the scope of the program;
  • Works to assure that systematic revisions to improve services are developed and implemented;
  • Facilitates linkages for clients and families between primary care, behavioral health providers, and other social service or provider agencies as needed to develop and coordinate service plans;
  • Collaborates with providers and others to conduct diagnostic screenings for substance abuse, mental health, co-occurring disorders, and triage referrals to appropriate waiver and/or treatment services;
  • Assists assigned clients with smooth transitions when moving into or out of the community;
  • Monitors and evaluates the effectiveness and outcome of treatment and service plans, and recommends modifications as necessary to provide optimal clinically appropriate services with a goal of maintenance in the community at the least restrictive level of care;
  • Attends case conferences, interagency, and provider treatment planning meetings for assigned clients;
  • Utilizes supervision with the MHW Care Manager Supervisor regularly;
  • Participates in professional development activities;
  • Attends annual Conflict of Interest training;
  • Attends training specific to job duties;
  • Maintains confidentiality of all client protected health information and adheres to all HIPAA-related policies and procedures;
  • Demonstrates ethical behavior and cultural sensitivity in all activities involving individuals of diverse backgrounds;
  • Performs other tasks/responsibilities as required to support business operations.

Requirements:

  • Master degree with active licensure in good standing in the state of Connecticut (LCSW, LPC, or LMFT), or license-eligible, and 3 years of behavioral health clinical experience;
  • Experience in the delivery of community-based services;
  • Any health-related requirements by Facility/Provider must be fulfilled;
  • General knowledge of best practices in behavioral health, emphasizing work with special needs or priority populations and in public sector systems.

  • Ability to collaborate well with various parties;
  • Enjoys autonomy and is adaptable to change;
  • Comfortable with independent problem-solving based on advanced-level knowledge of the service delivery system, the provider network, client services policies, clients' rights and responsibilities, and the operating practices of the organization;
  • Exercises strong clinical skills with a sophisticated understanding of the holistic needs of individual clients from a strengths-based perspective;
  • Ability to propose and implement creative solutions to client problems and to achieve a high level of client satisfaction with services;
  • Excellent written and verbal communication skills;
  • Demonstrated knowledge of clinical treatment and case management;
  • Knowledge of community resources;
  • Strong attention to detail; ability to work on multiple tasks and meet deadlines;
  • Excellent PC skills with demonstrated experience using Microsoft Office Package (MS Word, Excel, PowerPoint, Access, Outlook); internet; smartphone usage.
