Job Title: Intensive Care Manager

Job Summary:

An Intensive Care Manager is responsible for providing clinical leadership and coordination of recovery support and behavioral health services for clients with severe mental illness. The ICM works with clients, providers, and other stakeholders to develop and implement individualized recovery plans, ensuring a smooth transition between care settings and promoting optimal clinically appropriate services.

Key Responsibilities:

  • Assumes responsibility for a designated client case load, providing clinical expertise and knowledge to the team.
  • Travels statewide to assess and evaluate potential clients, completing all necessary paperwork and documentation.
  • Works collaboratively with providers and other stakeholders to develop and implement individualized recovery plans, ensuring a smooth transition between care settings.
  • Provides clinical supervision for LCSW candidates working towards licensure, ensuring they meet the necessary requirements and standards.
  • Conducts regular recovery plan reviews, service authorizations, and care coordination for all assigned clients, ensuring they receive the necessary support and services.
  • Maintains an understanding of behavioral health benefits, policies, and procedures, ensuring compliance with all relevant regulations and guidelines.
  • Supervises the collection of information regarding the delivery and outcomes of services to clients, using this data to recommend modifications to plan policies and procedures.
  • Provides clients, providers, and other stakeholders with accurate and timely information concerning waiver benefits and coverage, ensuring they understand their rights and responsibilities.
  • Works collaboratively with clients and providers to customize services to best meet client needs, ensuring a high level of client satisfaction with services.
  • Facilitates linkages for clients and families between primary care, behavioral health providers, and other social service or provider agencies as needed, ensuring a comprehensive and coordinated approach to care.
  • Collaborates with providers and others to conduct diagnostic screenings for substance abuse, mental health, co-occurring disorders, and triage referrals to appropriate waiver and/or treatment services.
  • Assists assigned clients with smooth transitions when moving into or out of the community, ensuring a seamless and supportive experience.
  • Monitors and evaluates the effectiveness and outcome of treatment and service plans, recommending modifications as necessary to provide optimal clinically appropriate services.
  • Attends case conferences, interagency, and provider treatment planning meetings for assigned clients, ensuring a collaborative and coordinated approach to care.
  • Utilizes supervision with the MHW Care Manager Supervisor regularly, ensuring they meet the necessary requirements and standards.
  • Participates in professional development activities, staying up-to-date on best practices and emerging trends in behavioral health.
  • Maintains confidentiality of all client protected health information and adheres to all HIPAA-related policies and procedures.
  • Demonstrates ethical behavior and cultural sensitivity in all activities involving individuals of diverse backgrounds.

Requirements:

  • Master's degree with active licensure in good standing in the state of Connecticut (LCSW, LPC, or LMFT), or license-eligible, and 3 years of behavioral health clinical experience.
  • Experience in the delivery of community-based services, with a focus on working with special needs or priority populations.
  • General knowledge of best practices in behavioral health, emphasizing work with special needs or priority populations and in public sector systems.

  • Ability to collaborate well with various parties, including clients, providers, and other stakeholders.
  • Enjoys autonomy and is adaptable to change, with a strong ability to problem-solve and think critically.
  • Comfortable with independent problem-solving based on advanced-level knowledge of the service delivery system, provider network, client services policies, client rights and responsibilities, and the operating practices of the organization.
  • Exercises strong clinical skills with a sophisticated understanding of the holistic needs of individual clients from a strengths-based perspective.
  • Ability to propose and implement creative solutions to client problems and achieve a high level of client satisfaction with services.
  • Excellent written and verbal communication skills, with the ability to communicate complex information in a clear and concise manner.
  • Demonstrated knowledge of clinical treatment and case management, with a focus on working with clients with severe mental illness.
  • Knowledge of community resources, with a focus on providing clients with access to necessary support and services.
  • Strong attention to detail, with the ability to work on multiple tasks and meet deadlines.
  • Excellent PC skills, with demonstrated experience using Microsoft Office Package (MS Word, Excel, PowerPoint, Access, Outlook), internet, and smartphone usage.
