Key Responsibilities- Provide one-on-one care to pediatric patients in their homes, promoting their physical, emotional, and developmental well-being.
- Develop and implement personalized care plans, working collaboratively with patients, families, and healthcare teams to ensure seamless care coordination.
- Administer medications, perform medical procedures, and provide wound care, as needed.
- Conduct thorough assessments, monitoring patients' conditions, and reporting any changes or concerns to healthcare teams.
- Collaborate with healthcare professionals to ensure continuity of care, sharing knowledge and expertise to optimize patient outcomes.
- RN/LPN with a current, valid license to practice as issued by the State Board of Nursing.
- Current CPR certification.
