Senior Weekend RN Position in Grand Rapids, Michigan

We are seeking a skilled Senior Weekend Registered Nurse to join our team at Trinity Health in Grand Rapids, Michigan. As a vital member of our Home Care Baylor team, you will provide exceptional care to patients who require one-to-one attention and monitoring in their homes.
About the RoleThis is a full-time weekend registered nurse position, working 3-12 hour shifts. As a Senior Weekend RN, you will be responsible for client assessment, care planning, intervention, and overall supervision of patient care and outcomes.
Responsibilities- Provide one-to-one care with patients in their homes, focusing on their unique needs and goals.
- Develop and implement personalized care plans, taking into account the patient's medical history, current condition, and preferences.
- Conduct thorough assessments, identify potential health risks, and recommend interventions to prevent complications.
- Collaborate with healthcare teams, including physicians, therapists, and other caregivers, to ensure seamless care coordination.
To succeed in this role, you must have:
- A graduate degree from an approved Nursing education program, preferably with a BSN.
- A valid Registered Nurse licensure in the state of Michigan.
- A minimum of one year of professional nursing experience, preferably in home healthcare or a related field.
- Strong computer and technology skills, as well as effective communication and interpersonal skills.
- A reliable vehicle and a valid driver's license.
We offer a competitive salary, with an estimated annual income of $85,000-$110,000, depending on experience. Our benefits package includes:
