Social Services Coordinator Position Summary
We are seeking a highly motivated Social Services Coordinator to join our team at Ibero American Action League. As a Social Services Coordinator, you will be responsible for providing care management services to program participants who require medical, behavioral health, and social services. Your primary goal will be to help these individuals access the resources they need to improve their overall well-being.

Responsibilities:
This role involves a variety of responsibilities, including:
* Identifying individuals in the community who may benefit from our care management services
* Contacting assigned participants within 3 days of receiving the assignment to assess their needs and develop a plan to address them
* Maintaining accurate and updated records of all service provided, including participant information, service plans, and outcomes

Requirements and Qualifications:
To be considered for this role, you must have a GED or Associate's degree in Human Services or a related field and at least 2 years of experience working with Latinos connecting them with community services. You should also be bilingual in Spanish and English, have basic knowledge of Microsoft Word, and be able to provide own transportation and have a valid good standing NYS driver's license. Additionally, you should possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Benefits and Compensation:
