Job Opportunity

Medical Services of America Home Health & Hospice is seeking an experienced Hospice Nurse Case Manager to join our team.

Responsibilities
  • Provide high-quality patient care by assessing, planning, implementing, and evaluating patient care plans in accordance with the plan of treatment signed by the physician.
  • Conduct regular evaluations of the patient's nursing needs and make necessary revisions to the patient's plan of care.
  • Initiate diagnostic, preventive, and rehabilitative nursing procedures as appropriate to the patient's care and safety.
  • Monitor and record patient reactions to treatment and any changes in the patient's condition.
  • Teach and supervise patients and families regarding home nursing procedures and other care needs as appropriate to the patient's condition.
Requirements
  • Completion of a nursing program from an accredited school or university.
  • Current licensure as a RN in the state of VA.
  • Minimum of one year nursing experience required.
  • Home Health experience strongly preferred.
  • Valid Driver's License required.
