Job Description

Job Summary:

We are seeking a skilled Health Coach to join our team at Med Employment Consulting. As a Health Coach, you will play a vital role in promoting wellness and preventative care, improving member quality of care and health outcomes, reducing hospitalization, and ensuring compliance with regulatory requirements.

Key Responsibilities:

  • Perform in-person care management visits to assess patients' health and well-being.
  • Assist with benefit coordination, order routine supplies, and troubleshoot supply and DME delivery issues.
  • Notify clinical staff of identified issues and clinical triggers requiring escalation.
  • Conduct at-home patient surveys and assessments.

Requirements:

  • Must be an independent contractor and able to receive a 1099.
  • Must be able to work on a fee-for-service basis.
  • Must be able to complete the following and provide copies:
    • Driver's license or government-issued ID
    • Copy of Social Security card
    • Application
    • Medical history complete with vaccination history
    • Copy of the voided check

Training and Development:

One-day training is mandatory and will consist of:

  • Infection control training
  • Paperwork/software training
  • Specific tests

Compensation:

Compensation for the 30-minute screening is $40-$70 depending on the number of tests administered.
