Case Manager

The Case Manager will provide relational support to low-income clients, working towards accomplishing their goals and family sufficiency. This role offers encouragement and resources to clients and is responsible for tracking and documenting the case status.
Key Responsibilities:
• Facilitate the case management of participants within program guidelines, utilizing assessment tools, and developing rapport and partnership with participants.
• Develop case plans and support participants in establishing goals, meeting regularly to track progress, and coordinating referrals to support desired program outcomes.
• Maintain working relationships with community agencies to maintain awareness of available services, promote The Salvation Army's services, and coordinate services and communicate about cases.
• Ensure that valid releases of information are on file and create case plans to help participants achieve identified goals through agency resources and referral to outside resources as available and appropriate.
• Enhance personal knowledge and skill through community and agency educational forums and use electronic databases to enter, retrieve, and maintain accurate case records per protocols.
• Manage service transactions, case plans, and assessments, monitor assistance fund balances, and request resources as appropriate.
• Fulfill ad hoc and periodic survey requests and provide information needed for reports to administration.
Additional Responsibilities:
• Oversee and manage Food Pantry operations, Pathway of Hope program, and seasonal assistance programs, such as Angel Tree, Back to School, etc.
Requirements:
• Bachelor's degree in social work, psychology, sociology, or related field, or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
• 2 or more years of applicant interview or case management experience, with experience working with program target population preferred.
• Learn and adhere to The Salvation Army Social Services Code of Ethics, professional codes of ethics, and professional boundaries within and outside of the organization.
• Maintain client confidentiality and obtain appropriate releases of information, complete Safe From Harm training, and keep current as needed.
• Demonstrate a continued effort to increase self-awareness and to better understand other cultures and issues of social justice and promote racial understanding and reconciliation.
• Complete Caseworker Certification Program within 120 days of hire.
Computer Skills:
• Proficient in Microsoft Office 365, working knowledge of TEAMS and SharePoint, and proficiency in SIMS (ServicePoint) software within 90 days.
• Willingness to learn new software as needed.
Certificates and Licenses:
• Current valid driver's license and pass the Salvation Army Motor Vehicle Record check (MVR).
