About the JobIn this role, you will have the opportunity to work in a dynamic and supportive environment, providing individualized patient care and collaborating with healthcare teams to develop and implement patient care plans.
ResponsibilitiesYour key responsibilities will include:
- Providing patient care in a home environment, including assessment, planning, implementation, and evaluation
- Executing physicians' prescribed plans of care, including administering medication and treatments
- Coordinating care with healthcare teams, including doctors, therapists, and other caregivers
To be successful in this role, you will need:
- A valid LPN license in the state where you will practice
- Current CPR certification
- The ability to communicate effectively with patients, families, and healthcare teams
We offer a competitive salary range of $25-$32 per hour, depending on experience and location. Additionally, you will receive:
