Job Summary

The Advanced Practice Nurse (APN) is a key member of our healthcare team, providing high-quality patient care and leading clinical initiatives at Albany Medical Center.

Key Responsibilities
  • Conduct comprehensive patient assessments, develop and implement individualized care plans, and coordinate with interdisciplinary teams to ensure seamless patient care.
  • Order diagnostic tests, interpret results, and make informed decisions about patient treatment, including prescribing medications and ordering therapies.
  • Collaborate with physicians, nurses, and other healthcare professionals to develop and implement evidence-based clinical practices and protocols.
  • Provide education and support to patients, families, and caregivers on disease management, self-care, and health promotion.
  • Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient outcomes and satisfaction.
Requirements
  • Master's Degree from an accredited Nurse Practitioner program.
  • Current certification as an Advanced Practice Nurse (APN) in a relevant specialty.
  • Excellent communication, interpersonal, and leadership skills.
  • Ability to work effectively in a fast-paced, dynamic environment.
  • Proficiency in electronic medical records and other healthcare technologies.
