Position Overview:
The Advanced Practice Nurse in our outpatient nephrology clinic plays a vital role in delivering comprehensive care to patients with kidney-related conditions, particularly Chronic Kidney Disease.
Working in collaboration with nephrologists and a multidisciplinary healthcare team, the NP is tasked with assessing, diagnosing, and managing the care of patients with diverse kidney ailments.
The emphasis is on providing high-quality, patient-focused care while engaging with the healthcare team.Key Responsibilities:
- **Patient Assessment:** Perform detailed evaluations of patients with kidney issues, including medical history reviews, physical examinations, and diagnostic testing.
- **Diagnosis and Treatment:** Develop accurate diagnoses based on assessments and work with the healthcare team to create and implement effective treatment plans.
- **Medication Management:** Prescribe necessary medications, monitor patient responses, and modify treatment plans as needed. Educate patients about medication management and potential side effects.
- **Patient Education:** Offer extensive education to patients and their families regarding kidney disease management, lifestyle changes, and preventive strategies.
- **Collaboration:** Work closely with nephrologists, nurses, dietitians, and other healthcare professionals.
- **Follow-up Care:** Schedule regular follow-up appointments to track patient progress, adjust treatment plans, and address any concerns or complications.
- **Documentation:** Ensure accurate and timely electronic health records (EHR) and documentation of patient assessments, diagnoses, treatments, and educational efforts.
- **Quality Improvement:** Engage in quality improvement initiatives aimed at enhancing care delivery, patient outcomes, and clinic efficiency.
Qualifications:
Current Texas Licensure as a Registered Nurse
Current Texas Licensure as an APRN-NP, with specialty certification as required by the department
Basic Life Support (BLS) Certification or specialty certification as required by the department
Education Requirements:
Master's degree in Nursing as NP or Master's degree in Nursing with completion of a post-master's Nurse Practitioner Certification Program.
Experience Requirements:
No prior experience required
Physical Demands:
Must be able to exert up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.
