We are seeking a skilled Nurse Practitioner to join our team at the Karen Ann Quinlan Memorial Foundation. As a Nurse Practitioner, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.
Job SummaryThe Nurse Practitioner will be responsible for providing primary and specialty care to patients in a variety of settings, including hospice and palliative care. This role requires a strong foundation in family practice, oncology, or hospital practice, with a focus on delivering compassionate and patient-centered care.
Key Responsibilities- Provides and documents medically necessary services in accordance with physician/NP protocol.
- Develops treatment plans that maximize patient health potential, including ordering and interpreting diagnostic tests, identifying appropriate pharmacologic agents, and developing education plans.
- Consults with collaborative physicians, consulting practitioners, primary care physicians, and other specialist physicians to ensure comprehensive patient care.
- Establishes relationships with hospital-based palliative care teams, partner organizations, medical directors, hospital physicians, community physicians, nurses, and other team members to build a culture of collaboration and clinically integrated care.
- Refers patients to medical specialists and/or other healthcare services as medically indicated, and assists in all facets of care coordination for referrals.
- Develops outreach programs for community agencies and healthcare settings, and participates in outreach presentations to interested groups.
- Provides disease management instruction and education to patients and their families, and performs on-call services as applicable.
- Participates in interdisciplinary team meetings, prepares and maintains accurate patient records, charts, and documents, and acts as a clinical resource to coordinate complex cases for safe and appropriate transitions to other care settings.
- Complies with applicable laws and regulations, reviews KAQ policies and services with referred patients and/or family caregivers or authorized patient representatives, and obtains consent for medical care.
- Works collaboratively with the Intake department to verify coverage/payment and initiates reimbursement for services rendered, and assists in the development and review of clinical practice guidelines/standards in support of quality care.
- Acts with integrity, compassion, and customer focus to ensure quality care, adheres to employment policies, maintains confidentiality, and adheres to all HIPAA regulations.
- Participates in all required in-service education programs and meetings, and demonstrates compliance with the dress code policy by appearing well-groomed, wearing an ID badge while on duty, and maintaining a professional appearance.
To be successful in this role, you will need:
- A Board-licensed nurse practitioner, licensed to practice in New Jersey (PA license may also be required depending on territory served).
- Hospice and Palliative Care Certification (can be obtained within 2 years, utilizing tuition reimbursement).
- Compliance with current department of Public Safety and maintenance of Drug Enforcement Agency registrations.
- DEA Licensure required.
- Experience in family practice, oncology, or hospital practice, with a focus on delivering compassionate and patient-centered care.
- Prior experience as a nurse practitioner is preferred.
