Responsibilities:- Conduct comprehensive evaluations and assessments of patients' communication and swallowing disorders
- Develop and implement individualized treatment plans to address patients' needs and goals
- Provide therapy sessions to patients in their homes, using a variety of techniques and modalities
- Collaborate with other healthcare professionals to ensure comprehensive care and coordination
- Document patient progress and outcomes in a timely and accurate manner
- Master's degree in Speech-Language Pathology from an accredited program
- Current licensure as a Speech-Language Pathologist in the state of Connecticut
- Experience working with patients with communication and swallowing disorders
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Experience working in home health or hospice settings
- Knowledge of medical terminology and coding systems
- Ability to use electronic medical records and other software applications
