Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the homes of our patients, identifying potential health risks and developing personalized care plans to address their needs.

Key Responsibilities

  • Conduct adult and geriatric assessments, including medical history, diagnosis, and treatment, in the patient's home
  • Develop and implement personalized care plans to address identified health risks and needs
  • Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure seamless care coordination
  • Provide education and support to patients and their families to promote healthy behaviors and disease management
  • Document all assessments and care plans in accordance with Matrix Medical Network's guidelines and protocols

Requirements

  • Master's degree in Nursing or related field
  • Current RN and NP licensure in the state of practice, with prescription authority or ability to obtain
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent communication and interpersonal skills

Travel Requirements

  • Ability to travel to patient homes and other community settings
  • Valid state driver's license and reliable transportation
