Job Title: Licensed Clinical Social Worker

We are seeking a highly skilled Licensed Clinical Social Worker to join our team at Southern Indiana Community Healthcare.

Job Summary

The Licensed Clinical Social Worker will provide behavioral health services to patients, including assessment, diagnosis, and treatment planning. The ideal candidate will have experience working in a primary care setting and be proficient in social work theory and methods.

Responsibilities
  • Provide behavioral health services to patients, including assessment, diagnosis, and treatment planning.
  • Develop and implement treatment plans in collaboration with patients and other healthcare professionals.
  • Document patient interactions and treatment plans in a clear and concise manner.
  • Meet with patients to discuss their treatment plans and progress.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
Requirements
  • Master's degree in Social Work (MSW) or related field.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) in the State of Indiana.
  • Experience working in a primary care setting.
  • Proficiency in social work theory and methods.
  • Excellent communication and interpersonal skills.
