Job Summary

We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Weekend Hospice Nurse. This role involves providing care and services to a group of hospice patients, visiting patients in their homes, assisted living facilities, and hospitals as needed.

Responsibilities
  • Complete hospice admissions and patient visits as needed
  • Provide symptom management, deaths, and admissions as needed
  • Answer calls from patients, families, and customers
  • Work collaboratively with nurses, social workers, chaplains, volunteers, and CNA leadership
  • Manage visits, documentation, and meetings, prioritizing and triaging multiple concerns
Requirements
  • Current Registered Nurse (RN) licensure in good standing in the state(s) of practice with one year of experience under license
  • Admissions and on-call experience preferred
  • Ability to work independently and as part of a team
  • Strong communication and time management skills
  • Ability to learn an electronic medical records system
