As an Outpatient Coordinator, you will play a crucial role in ensuring the smooth operation of our programs, ensuring compliance with all laws, rules, and regulations, as well as maintaining a quality patient experience.
Key Responsibilities
• Monitor and complete necessary audits for patient care tracking
• Handle all aspects of the admission process, including supporting the completion of pre-admission paperwork as applicable
• Track and collect required assessments and surveys, such as BASIS-32/CABA and patient satisfaction surveys
• Maintain supply of therapeutic tools, such as worksheets, videos, and games, for clinician use
• Support the therapist in maintaining the programming schedule and charge reconciliation
• Assist with patient group assignments and coordination efforts, as well as vital and UDS capture as needed
• Complete DCAR reporting in a timely and accurate fashion
• Maintain staff schedules and ensure staffing for the program, monitoring and recording attendance, including tardiness and absences
• Support in organizing and filing paperwork, as well as auditing charts and communicating with the treatment team to ensure chart compliance
• Manage Family Session Schedules as required by the program
• Provide transportation daily via hospital vehicle to patients to/from home into the IOP Program
• Work with the business office and utilization review department to ensure payment for services, performing pre-certifications and concurrent reviews, and developing relationships with payer sources
• Maintain program and patient information and data to create reports, census, and assist the Business Office in reporting
• Maintain a positive working relationship with referral sources, community agencies, and organizations
• Schedule CPEs, nursing assessments, H&P, and psychosocial assessment with the appropriate clinician and per required timeframe
• Ensure orders are obtained for patients prior to starting services, as applicable
• Actively participate in the therapist's discharge planning needs, creating appointments, starting follow-up care process, and completing interfacing with organizations
• Cross-train and support in various outpatient functions, including PCA, transportation, and financial counseling
• Conduct appointment reminder calls and collect copays and deductibles as applicable
• Manage organizational tools to ensure efficient operations, including bed boards/group room assignments, treatment team schedule, flash reports, etc.
• Provide impeccable customer service to patients, families, referral sources, and stakeholders
Qualifications
• Education/Licensure: High school degree or equivalent
• Valid driver's license
• CPR certification and Crisis Prevention Training (CPI) within 30 days of employment and prior to any patient contact
