Key Responsibilities:

  • Provide high-quality primary care to our senior patients
  • Work collaboratively with our team of healthcare professionals to deliver exceptional care
  • Participate in quality improvement initiatives to drive excellence in patient care
  • Stay up-to-date with the latest medical knowledge and best practices

Requirements:

  • Current licensure to practice professional nursing or physician assistant in the state of employment
  • Active and non-probationary status
  • Experience in primary care, including internal medicine, geriatrics, or family medicine
  • Comfortable managing a primarily Medicare-aged, medically underserved patient population with comorbid conditions
