Job Summary

We are seeking a skilled Case Manager to join our team at Interfaith Community Services. As a key member of our Permanent Supportive Housing team, you will provide direct services to clients, including outreach, assessment, crisis intervention, advocacy, and case management. Your goal will be to support clients in achieving self-sufficiency and maintaining permanent housing.

Key Responsibilities
  • Provide direct services to clients, including outreach, assessment, crisis intervention, advocacy, and case management.
  • Complete documentation, including case notes, treatment plans, progress notes, and other required statistics.
  • Offer guidance and support to help clients identify their strengths, set achievable goals, and work towards self-sufficiency.
  • Administer supportive services in accordance with agency and contract guidelines.
  • Link and network clients with resources within the agency and other public and private social service agencies as necessary.
  • Support the community by working with clients in various settings, including other agency programs, faith centers, and school administrations.
  • Gather data for program reports for the Program Manager.
  • Familiarity with curriculum/training for Housing First, Trauma-Informed Care (TIC), and Harm Reduction.
  • Facilitate and/or coordinate monthly resident meetings, social events, classes (Life Skills, Active Recovery, etc.) to engage clients according to grants/contracts.
  • Read and follow department and agency policies and procedures, and participate in all staff meetings and trainings.
Requirements
  • Master's Degree in Social Work, Counseling, Marriage Family Therapy, or related field.
  • 1+ years of work experience in non-profit, including internships (housing, public, social benefit/services, health, etc.).
Preferred Skills, Experience, and Education
  • Bilingual Spanish/English.
  • Previously/Currently working with the homeless population.
