Job Title: LVN Charge Nurse

Regency Integrated Health Services is seeking a skilled LVN Charge Nurse to join our team at Memorial City Nursing and Rehabilitation Center in Houston, TX.

Job Summary

The LVN Charge Nurse will play a vital role in ensuring the delivery of high-quality care to our residents. As a leader on our nursing team, you will be responsible for overseeing the care of our residents, collaborating with our interdisciplinary team, and promoting a culture of excellence and compassion.

Key Responsibilities
  • Assess and document resident conditions, identifying areas for improvement and implementing changes as needed
  • Develop and implement individualized care plans, ensuring that each resident receives the highest level of care
  • Administer medications as prescribed, maintaining accurate records and ensuring compliance with facility policies
  • Collaborate with our interdisciplinary team to ensure seamless care transitions and effective communication
  • Supervise and mentor our LVN team members, providing guidance and support to ensure they have the resources and training needed to excel in their roles
  • Identify and report any changes in resident condition, taking necessary action to ensure their safety and well-being
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices and policies
Requirements
  • Active Licensed Vocational Nurse license in the state of Texas
  • Experience working in a skilled nursing facility, preferably in a leadership role
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities with ease
