Job Title: Hospice Registered Nurse Case Manager

We are seeking a compassionate and skilled Hospice Registered Nurse Case Manager to join our team at Resource 1 Homecare Staffing. As a Hospice RN Case Manager, you will play a vital role in providing high-quality care to patients and their families in their homes and communities.

Job Summary:

The Hospice Registered Nurse Case Manager is responsible for planning, organizing, and directing hospice care services. This includes conducting comprehensive assessments of patients and families, developing individualized care plans, and coordinating with healthcare teams to ensure seamless care delivery.

Responsibilities:
  • Conduct initial and ongoing comprehensive assessments of patients and families to determine hospice needs.
  • Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and families.
  • Coordinate with healthcare teams, including physicians, therapists, and social workers, to ensure comprehensive care delivery.
  • Provide education and support to patients and families on disease management, symptom control, and end-of-life care.
  • Collaborate with interdisciplinary teams to develop and implement care plans that meet the unique needs of each patient.
  • Document patient progress and outcomes in accordance with regulatory requirements and organizational policies.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
  • Graduation from an accredited school of nursing.
  • Current RN licensure in the state of practice.
  • 1-2 years of recent acute care experience in an institutional setting (required). 1-2 years of recent home care experience (preferred).
  • Valid driver's license and insured automobile.
  • Ability to work efficiently with minimal supervision and prioritize multiple tasks and responsibilities.
  • Excellent communication and interpersonal skills.
