Position Summary:
The role involves delivering direct patient care, addressing both emotional and physical needs as per the care plan, medical directives, and nursing protocols. The patient demographic spans from infants to seniors, with care settings including inpatient facilities, outpatient services, home health, or hospice environments.
Key Responsibilities:
- Assists in formulating and maintaining a personalized, goal-oriented care plan, collaborating with other healthcare professionals to ensure the needs of patients and caregivers are effectively met.
- Monitors, evaluates, documents, and communicates patient responses to treatments and educational efforts, adjusting care priorities based on patient and caregiver requirements. Collects and reports data regarding the patient's physical, psychological, environmental, self-care, educational, and discharge planning needs to the responsible physician or registered nurse.
- Engages in departmental enrichment initiatives that promote positive outcomes, including participation in committees, quality improvement activities, and the training of new staff.
- Administers a variety of treatments and therapies to patients, which may include respiratory care, suctioning, tracheostomy management, medication administration, cardiac monitoring, and assisting physicians with specialized procedures. Also responsible for monitoring intravenous therapies, conducting point-of-care testing, performing routine lab work, and providing patient and caregiver education based on established plans.
- Supports patients and families during their admission process, orientation to the healthcare experience, and discharge planning.
- Offers emotional support to patients and families, employing de-escalation techniques for anxious or potentially aggressive individuals, and facilitating coping strategies appropriate for their age.
Educational Qualifications:
Completion of a state-approved practical nursing program is required.
Licensure/Certification:
Must hold a valid Practical Nurse (LPN) license in North Carolina. Basic Life Support (BLS) certification is also required.
Experience Requirements:
No prior experience is necessary.
