POSITION SUMMARY:

The Oncology Care Coordinator plays a pivotal role in the management of patients diagnosed with cancer. This position involves working collaboratively within a multidisciplinary team to identify and support high-risk patient populations, ensuring they receive comprehensive and coordinated care throughout their treatment journey.


KEY RESPONSIBILITIES:
  • Facilitate patient navigation through the healthcare system, addressing barriers to care and enhancing patient experience.
  • Provide education and resources to patients and their families regarding treatment options and care plans.
  • Monitor patient progress and outcomes, adjusting care plans as necessary to meet individual needs.
  • Collaborate with healthcare providers to ensure continuity of care and effective communication among all parties involved.

QUALIFICATIONS:

To be considered for this role, candidates should possess:

  • A Bachelor’s degree in Nursing (BSN) is required.
  • A minimum of 3-5 years of nursing experience, with a focus on oncology preferred.
  • Current Maryland Registered Nurse License.
  • Oncology Nurse Certification (OCN) is mandatory.
  • American Heart Association CPR Certification.
  • Experience in an outpatient setting is advantageous.
