Job Summary:

At Jewish Family Service Association of Cleveland, we are seeking a skilled Occupational Therapist to join our team on a PRN basis. As an Occupational Therapist, you will provide comprehensive, professional, and compassionate therapy services to clients in their homes. Your expertise will be invaluable in assessing and treating clients according to their individualized plan of care, monitoring and documenting their condition, progress, and outcomes, and providing education and support to clients and caregivers.

Key Responsibilities:

  • Conduct OT assessments, diagnostic tests, and skilled treatments to clients receiving services under a medically approved Plan of Care.
  • Participate in the development of the Plan of Care.
  • Guide and instruct clients in therapeutic self-care and creative activities to improve independence and physical and cognitive functioning.
  • Prepare reports of diagnostic evaluations and make recommendations.
  • Prepare clinical and progress notes.
  • Instruct clients and their families in Activities of Daily Living.
  • Maintain confidentiality in all aspects of the job.
  • Attend care conferences as deemed necessary.
  • Participate in in-service programs as requested.

Requirements:

Applicants must be licensed in the state of Ohio and be a graduate of a recognized and accredited occupational therapy program. Home health agency or experience in a healthcare facility is required. Current occupational therapy license, CPR certification, and a valid Ohio Driver's License are also necessary.
