Nutritionist Job Description

At Patient Centered Services, we are seeking a highly skilled Nutritionist to join our team. As a Nutritionist, you will work collaboratively with healthcare providers to assess, plan, and implement individualized nutritional plans for patients.

Key Responsibilities:
  • Conduct comprehensive nutritional assessments for patients, identifying specific dietary needs based on health conditions, allergies, and lifestyle.
  • Develop and implement customized nutrition plans tailored to individual patient needs and medical conditions.
  • Monitor patients' progress, adjust nutrition plans as necessary, and provide ongoing support and follow-up.
  • Educate patients and their families on proper nutrition, healthy eating habits, and the role of diet in managing chronic illnesses such as diabetes, cardiovascular diseases, and obesity.
  • Collaborate with physicians, nurses, and other healthcare providers to create holistic care plans.
  • Provide nutrition counseling to patients, offering strategies for weight management, improving energy levels, and optimizing overall health.
  • Create educational materials on nutrition and healthy eating for patient distribution.
  • Stay current with the latest nutrition research and integrate evidence-based practices into patient care.
  • Document patient encounters and progress in the electronic medical record (EMR) system.
  • Participate in interdisciplinary team meetings to discuss patient progress and contribute to comprehensive care plans.
  • Advocate for healthy eating and nutrition awareness through community outreach programs and events.
Requirements:
  • Bachelor's degree in Nutrition, Dietetics, or a related field from an accredited institution.
  • Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) certification.
  • Active state license to practice nutrition in Washington, DC.
  • Minimum of 2 years of clinical experience in nutrition counseling, preferably in a healthcare or home health setting.
Preferred Qualifications:
  • Master's degree in Nutrition or Dietetics.
  • Experience working with specific populations, such as pediatric, geriatric, or chronically ill patients.
  • Multilingual abilities (especially English/Spanish) are preferred but not required.
Compensation:

$35.00 - $40.00 per hour
