Responsibilities

The Primary Care Physician Assistant we seek is a highly skilled and compassionate healthcare professional with experience in home health, geriatrics, hospice, and primary care.

Responsibilities include:

  • Providing primary care to high-acuity patients in their homes, Monday through Friday
  • Working collaboratively with a support team to ensure seamless care delivery
  • Developing and implementing personalized care plans that address the unique needs of each patient
  • Staying up-to-date with the latest medical research and best practices in home-based primary care

Qualifications

To be considered for this role, you must be a licensed NP in the state where HarmonyCares Medical Group is located, have a federal DEA license/eligibility, and possess geriatric training/experience. Active CPR/BLS certification is also required.
