Job Title: Registered Nurse - Field Assessor

LHC Group is seeking a skilled Registered Nurse to join our team as a Field Assessor. As a Field Assessor, you will be responsible for conducting assessments in the comfort of the patient's own home, evaluating their functional level of independence, and documenting their findings.

Responsibilities:
  • Conduct thorough assessments of patients in their homes, evaluating their ability to perform daily living activities and complete instrumental activities of daily living.
  • Document patient information, including demographics, physician information, medication lists, and functional assessments, using a point-of-care web application.
  • Utilize clinical reasoning and judgment to assess patients' cognitive function and ability to complete daily tasks.
  • Work independently, using a laptop or tablet to complete assessments and document findings in real-time.
Requirements:
  • Current, unencumbered RN licensure in the state of practice.
  • Minimum of 2 years of home health experience or 2 years of nursing experience in an equivalent area of practice serving the older adult population.
  • Ability to obtain a professional liability insurance policy.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
