LVN Charge Lead

We are seeking a highly skilled and compassionate LVN Charge to join our team at Family Health Centers of San Diego. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and ensuring the smooth operation of our clinics.
Key Responsibilities- Participate in LEAN/HONSHA trainings and serve as a member of the Core Team.
- Actively participate in clinical quality outcomes initiatives to achieve goals and metrics.
- Build positive relationships with staff, inspire others to perform at a higher level, and prepare career development plans.
- Coach and counsel staff, administer disciplinary action, and communicate effectively with the interdisciplinary care team.
- Complete required documentation accurately and thoroughly, ensuring subordinate staff complete documentation per department standards.
- Consistently follow Policies and Procedures to ensure positive patient outcomes.
- Keep clinic workflow moving, focusing on productivity and efficiency, and ensure site compliance with TJC, DHS, CHDP, and Medi-Cal Managed Care requirements.
- Participate in continuous improvement activities and ensure Lean tools are deployed and results are evaluated.
- 3 years of experience in an ambulatory care setting or similar setting preferred.
- California State Board LVN License required.
- Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.
- Graduation from a recognized practical nursing program.
- High School Graduate (or GED equivalent).
