We are seeking a dedicated and compassionate Part Time Nurse to join our team at Methodist Hospital Metropolitan. As a Part Time Nurse, you will play a vital role in delivering exceptional patient-centered care and ensuring a seamless transition for patients coming in and out of acute care hospital settings.

About the Role

As a Registered Nurse for Admission and Discharge, your top priority will be delivering exceptional patient-centered care. You will be dedicated to ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings. Your specialized assessment skills will help ensure that patients are placed in the appropriate care areas and discharged.

Responsibilities
  • Obtains a nursing assessment history, and ensures proper inpatient care unit admission.
  • Takes appropriate action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.
  • Completes and facilitates the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
  • Teaches patients/families/caregivers about a patient's medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures.
  • Translates complex medical terminology to ensure complete knowledge.
  • Completes extra discharge documentation, education, and discharge care for special patient populations, to ensure compliance with core measures for discharge.
  • Ensures patients /families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc.
  • Coordinates with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
Requirements

We are looking for a Registered Nurse with a Registered Nurse Diploma, or Associate Degree, or Bachelors Degree, and 1-2 years of Admission Discharge Lounge experience or Medical Surgical/Tele experiences.
