Job Summary

We are seeking a skilled Nurse Practitioner to join our Palliative Care team. As a Palliative Care Nurse Practitioner, you will work collaboratively with our team to deliver high-quality, patient-centered care to individuals with serious illnesses. Your expertise will be essential in developing and implementing individualized treatment plans, ensuring that our patients receive the best possible care.

Responsibilities
  • Assess patients' needs and develop comprehensive treatment plans in collaboration with the Palliative Medical Director and other team members.
  • Provide diagnosis, treatment, consultation, and follow-up care to patients referred to palliative care.
  • Work autonomously in an Advanced Practice Role, making independent decisions and taking responsibility for patient care from initial assessment through discharge.
  • Participate in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner.
  • Prescribe medications, including controlled substances, and order laboratory and diagnostic tests as needed.
  • Communicate clinical findings to the Palliative Medical Director and maintain effective working relationships with the supportive care team.
Requirements
  • Master's degree in Nursing from an accredited school of nursing.
  • Minimum of two years of Nurse Practitioner experience, with a specialty in Family or Adult Medicine preferred.
  • Current license as a registered nurse and Nurse Practitioner certification/license.
  • Collaborative agreement with a supervising physician or Palliative Medical Director, as required by state law.
  • Part B Provider certification and Federal DEA license.
