Job SummaryWe are seeking a skilled Nurse Practitioner to join our Palliative Care team. As a Palliative Care Nurse Practitioner, you will work collaboratively with our interdisciplinary team to provide comprehensive care to patients with serious illnesses. Your expertise will be essential in developing and implementing individualized treatment plans, managing symptoms, and improving the quality of life for our patients.
Responsibilities- Assess patients' physical, emotional, and spiritual needs, and develop comprehensive care plans to address these needs.
- Provide diagnosis, treatment, and follow-up care to patients, under the direction of the Palliative Medical Director.
- Collaborate with the Palliative Care team to deliver high-quality, patient-centered care.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Participate in ongoing education and training to stay current with best practices in palliative care.
- Master's degree in Nursing from an accredited school of nursing.
- Minimum of two years of experience as a Nurse Practitioner, preferably in palliative care or a related field.
- Current license as a registered nurse and Nurse Practitioner certification/license.
- Collaborative agreement with a supervising physician, as required by state law.
- Part B Provider certification and DEA license.
