Role Summary:
The Community Support Specialist plays a crucial role in enhancing the quality of life for Veterans facing neurological challenges, physical limitations, and traumatic brain injuries. This position is dedicated to delivering exceptional home and community-based services. The responsibilities include:
  • Implementing and supporting comprehensive care plans tailored to individual client needs.
  • Engaging effectively with clients and their families to educate them about available health care services and procedures.
  • Ensuring thorough and accurate documentation to facilitate timely invoicing.
  • Collaborating with Case Managers and family members to provide a holistic approach to care.
Key Responsibilities:
  • Participating in community events and activities.
  • Facilitating outdoor and recreational activities.
  • Assisting clients with organizational tasks, cognitive engagement, medical appointments, and household management.
  • Driving clients as needed.
  • Physical capability to lift up to 50 lbs.
  • Engaging in walking activities with clients.
Essential Requirements:
  • Current Resume
  • CPR/AED/First Aid Certification (from recognized organizations)
  • Social Security Card
  • Valid Driver's License
  • Auto Insurance (minimum coverage of 100k/300k for Bodily Injury)
