Role Summary:
The Community Support Specialist plays a vital role in enhancing the quality of life for Veterans facing neurological challenges, physical disabilities, and traumatic brain injuries. This position is dedicated to delivering exceptional home and community-based services. The specialist will effectively implement and support individualized care plans, ensuring that resources are utilized efficiently to address client needs. Responsibilities include educating clients and their families about available services, maintaining accurate documentation, and providing timely information for invoicing purposes. Collaboration with Case Managers and family members is essential to provide comprehensive support through various social, recreational, wellness, educational, and life skills training activities.

Key Responsibilities:
  • Participate in community engagement activities
  • Facilitate outdoor recreational events
  • Assist with planning, cognitive support, medical appointments, and household management
  • Transport clients as needed
  • Ability to lift up to 50 lbs
  • Engage in physical activities such as walking
Essential Requirements:
  • Current Resume
  • CPR/AED/First Aid Certification (from recognized organizations)
  • Valid Social Security Card
  • Driver's License
  • Auto Insurance (minimum coverage of 100k/300k for bodily injury)
