About the Role

We are seeking a highly skilled Foster Care Placement Specialist to join our team at Health Connect America, Inc. This is a unique opportunity to make a meaningful impact in the lives of children and families.

Key Responsibilities
  • Maintain in-depth knowledge of available resources and the child welfare system to match children's needs with the most suitable placements.
  • Collaborate with Child Protective Services and case management organizations to complete intake screenings and gather comprehensive information about children.
  • Document all efforts in matching children to the most appropriate homes, considering factors such as siblings, school, removal area, and individual needs.
  • Work in partnership with licensing teams, agencies, and residential providers to secure the most suitable placements for children in care.
Requirements
  • Minimum of a bachelor's degree in social work or a closely related field.
  • At least one year of experience working with special needs children in a child welfare setting.
  • Relevant case management experience or experience with foster care is preferred.
  • Child Welfare Certification is a plus.
