About the Role

We are seeking a skilled and compassionate Home Health Speech Language Pathologist to join our team at Interim HealthCare. As a Home Health Speech Language Pathologist, you will play a vital role in providing personalized speech therapy to patients in the comfort of their own homes.

Key Responsibilities
  • Conduct home-based speech therapy sessions to help patients overcome communication, swallowing, or cognitive difficulties.
  • Work collaboratively with a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, PTs, and OTs, to ensure comprehensive patient care.
  • Evaluate patient progress, document outcomes, and adjust treatment plans as needed to ensure patients meet their goals.
  • Develop and implement individualized therapy plans to address patients' unique needs and challenges.
  • Provide education and support to patients and their families on speech therapy goals, safety practices, and self-care techniques.
Requirements
  • Master's or Doctoral degree in Speech-Language Pathology and active SLP license in Colorado.
  • Minimum of 1 year of direct client care speech therapy experience in the last 5 years, preferably in home healthcare.
  • CPR Certification (demonstration course required).
  • Knowledge of state and federal home health regulations.
  • Excellent clinical judgment, critical thinking skills, strong communication, empathy, and compassion.
