We are currently seeking a Licensed Practical Nurse to join our team and make a meaningful difference in the lives of our clients. As a member of our team, you will have the opportunity to work independently, free from the limitations of a large hospital or long-term care facility.

Responsibilities include:

  • Providing professional nursing care as defined in the nurse practice act and under the direction of a RN.
  • Communicating appropriate status/condition changes to the RN Clinical Care Manager or designated RN.
  • Providing skilled nursing care for clients as directed by the RN Clinical Care Manager.
  • Following the Plan of Care and Nursing Care Plans established by the physician and RN.
  • Recognizing and reporting changes in client condition to the RN Clinical Care Manager and physician, as directed.

Requirements include:

  • Valid Indiana Nursing License
  • Valid driver's license
  • Willingness to travel to nearby clients in need of assistance
