Job Title: Home Health Care Registered Nurse

Job Summary:

We are seeking a skilled and compassionate Registered Nurse to join our team at Choice Home Health. As a Home Health Care Registered Nurse, you will provide high-quality patient care in the comfort of their own homes. Our unique focus on patient-centered care and evidence-based medicine empowers our patients to achieve their optimal level of independence.

Responsibilities:

* Perform comprehensive assessments in patients' homes and identify their needs
* Develop a plan of care in collaboration with patients, caregivers, healthcare providers, and other disciplines as needed
* Assess the need for additional services and equipment and inform the case manager and/or supervisor on a timely basis
* Take verbal orders from healthcare providers as necessary and document appropriately
* Initiate appropriate preventive, rehabilitative, therapeutic, comfort, and pain interventions and provide instructions to patients and caregivers
* Demonstrate professionalism by maintaining high standards of clinical care and staying current on clinical and medication issues

Qualifications:

* Graduation from an accredited nursing program and current state license as a Registered Nurse
* At least one year of clinical practical nursing experience, with home health experience preferred
* Must have a vehicle and current driver's license and automobile insurance
* Interpersonal and communication skills necessary
