Aveanna Healthcare LPN Full-Time Weekdays

**Job Summary:**
We are seeking a dedicated and compassionate LPN to join our team at Aveanna Healthcare. As a LPN, you will play a vital role in providing high-quality patient care and support to our clients.
Responsibilities:
- Provide skilled nursing care to clients in their homes, focusing on their physical, emotional, and social needs.
- Develop and implement individualized care plans in collaboration with clients, families, and healthcare teams.
- Conduct thorough assessments, identify areas of concern, and make recommendations for care.
- Administer medications, perform wound care, and provide other treatments as prescribed by physicians.
- Document client progress, identify areas for improvement, and make recommendations for future care.
- Collaborate with healthcare teams to ensure seamless care transitions and coordination.
Requirements:
- Active, unencumbered LPN license in the state where you will practice.
- Current CPR certification from American Red Cross or American Heart Association.
- TB skin test (current within the last 12 months).
- Six months prior hands-on nursing experience preferred but not required.
- Reliable transportation.
