Private Duty Nurse LPN Full-Time Weekdays

Aveanna Healthcare is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality patient care in the comfort of their own homes. As a Private Duty Nurse, you will be responsible for delivering one-on-one care to patients with complex medical needs, working closely with our interdisciplinary team to ensure seamless care coordination.
Key Responsibilities- Implement and manage patient care plans in accordance with physician orders and Aveanna's policies and procedures.
- Conduct thorough patient assessments, identify care needs, and develop individualized care plans to meet those needs.
- Administer medications, treatments, and therapies as prescribed by physicians, ensuring accurate and timely documentation in our electronic medical record system.
- Provide education and training to patients and their families on disease management, medication administration, and other relevant health topics.
- Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care coordination and communication.
- Participate in ongoing quality improvement initiatives to enhance patient outcomes and satisfaction.
- Current, unencumbered LPN licensure in the state where you will be practicing.
- At least 6 months of hands-on nursing experience, preferably in a home health or hospice setting.
- Current CPR certification from the American Red Cross or American Heart Association.
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
- Reliable transportation and a valid driver's license.
