Job Overview

Amazing Care Home Health Services is seeking a dedicated LPN IHSS Case Manager to provide exceptional in-home care services to clients across Colorado.


Key Responsibilities:

  • Deliver high-quality, compassionate care to clients in their homes, ensuring their safety, comfort, and well-being.
  • Collaborate with supervising RNs to develop and implement individualized care plans based on client needs and preferences.
  • Conduct regular client visits within associated branch counties, demonstrating reliability and flexibility in scheduling and travel.
  • Utilize Kantime software for accurate and timely documentation of client care activities and progress.
  • Maintain open communication with clients, families, and interdisciplinary team members to coordinate care and address any concerns or changes in condition.

Requirements:

  • Current LPN license in the state of Colorado
  • Minimum of 2 years of experience in a home health care setting.
  • Experience working with both pediatric and adult clients preferred.
  • Reliable transportation and willingness to travel within associated branch counties.
  • Ability to work collaboratively with supervising RNs and other team members.
  • Familiarity with Kantime software is a plus.
  • Knowledge of IHSS regulations and procedures is desirable.
  • Flexibility to commute from Loveland to Westminster areas is advantageous.
  • Willingness to pick up skilled visits within designated areas as needed.
