Key Responsibilities- Provide outreach and education to the community about substance use disorder and the benefits of the program.
- Conduct assessments and develop case plans for participants, ensuring they receive necessary support and resources.
- Establish partnerships with local service providers and agencies to ensure participants receive comprehensive care.
- Coordinate with VOA programs and other service providers to provide recovery-related support and services.
- Monitor participant progress and make adjustments to case plans as needed.
- Bachelor's degree in social work or related field, or at least five years of experience in the field and no degree.
- Excellent communication and interpersonal skills, with the ability to work with diverse populations.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Valid driver's license and reliable transportation.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
