Job Title: Licensed Independent Social Worker

MPAC Healthcare is a leading provider of post-acute care services, and we are seeking a highly skilled and motivated Licensed Independent Social Worker (LISW) to join our team. As a LISW, you will play a critical role in delivering high-quality mental health services to our residents, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.

Job Summary:

The successful candidate will have a strong background in counseling and therapy, with experience working with older adults in a skilled nursing facility setting. You will be responsible for providing initial assessments, utilizing psychotherapy to address mental health issues, and offering ongoing support to our residents. You will also be responsible for all documentation requirements and staying up-to-date on Medicare Benefit and commercial insurance plans.

Key Responsibilities:
  • Provide high-quality mental health services to residents, including initial assessments, psychotherapy, and ongoing support
  • Collaborate with the interdisciplinary team to ensure the best possible outcomes for residents
  • Document all interactions with residents and maintain accurate records
  • Stay up-to-date on Medicare Benefit and commercial insurance plans
  • Participate in ongoing education and training to enhance clinical skills and knowledge
Requirements:
  • Master's degree in Social Work and a valid LISW license in the state for which you are applying
  • Two years of counseling and therapy experience, preferably in a skilled nursing facility setting
  • Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals
  • Ability to work autonomously and as part of a team, with a strong commitment to delivering high-quality care
  • Knowledge of mental health diagnoses and therapeutic approaches, with a focus on older adults
