As a key member of our healthcare team, you will be responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating services across the continuum of care to ensure quality patient outcomes and appropriate utilization of healthcare services.
Key Responsibilities:- Assess patient needs and develop individualized care plans to ensure seamless transitions between care settings.
- Collaborate with healthcare teams to coordinate patient care, including discharge planning and post-acute care services.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and care coordination.
- Participate in quality improvement initiatives to enhance patient care and satisfaction.
- Graduate of an accredited school of nursing.
- Current licensure as a Registered Nurse in the state of Indiana.
- Two years of recent clinical experience or three years of recent hospital case management experience.
- Certification in Case Management (CCM or ACMA) preferred.
- Strong organizational and problem-solving skills, with ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
