About the Role
Kaiser Permanente is seeking a dedicated and compassionate RN - Primary Care (Mt. Scott / 36 Hr / Days) to join our team in Clackamas, Oregon. As a member of our healthcare team, you will play a vital role in delivering high-quality care to patients and promoting their overall health and well-being.
Job Summary
This position requires a registered nurse with at least two years of experience in primary care or a related field. The ideal candidate will possess excellent clinical skills, leadership abilities, and strong communication skills. They will work collaboratively with other healthcare professionals to provide patient-centered care and ensure seamless transitions across different care settings.
Key Responsibilities
- Provide direct patient care, including assessing patient needs, developing individualized care plans, and implementing evidence-based treatments.
- Collaborate with multidisciplinary teams to coordinate care, manage patient flow, and optimize outcomes.
- Communicate effectively with patients, families, and healthcare providers to promote continuity of care and address patient concerns.
- Participate in quality improvement initiatives to enhance patient safety, satisfaction, and outcomes.
- Maintain accurate and timely documentation of patient information, adhering to relevant laws, regulations, and organizational standards.
Requirements
- Minimum two years of RN experience within the last five years OR successful completion of the Kaiser Permanente NW Ambulatory RN New Grad/transition to practice program.
- Graduate of Accredited School of Nursing.
- High School Diploma or General Education Development (GED) required.
- Registered Nurse License (Washington) within 6 months of hire OR Compact License: Registered Nurse within 6 months of hire.
- Registered Nurse License (Oregon) within 6 months of hire.
- Pediatric Advanced Emergency Assessment, Recognition and Stabilization within 6 months of hire.
- Basic Life Support required at hire.
Preferred Qualifications
- Previous experience with population care/case management, triage, and advice.
- Current or future bachelors degree in nursing or related field.
Salary Information
The estimated annual salary for this position is $85,000-$110,000, depending on experience and qualifications.
