About the Role:The Registered Nurse will be responsible for providing direct and indirect comprehensive nursing care to patients with terminal illnesses. This includes:
- Providing prescribed medical treatment and personal care services to patients in their homes or inpatient settings.
- Completing clinical assessments and evaluations to determine the level of care required.
- Participating in interdisciplinary team meetings to develop and implement patient care plans.
- Collaborating with facility medical case managers to ensure smooth patient transitions.
To be successful in this role, you will need:
- A diploma, Associate's or Bachelor's degree in Nursing from an accredited program.
- At least 1 year of experience in hospital settings, with a focus on hospice, home care, and community health.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- A valid driver's license and Registered Nurse license in the State of New Jersey.
