Job Overview:
The LPN Clinical Associate plays a crucial role in assisting the Resident Care Manager with overseeing the Resident Care Aides, providing medication support, facilitating daily living activities, and assessing the medical and clinical requirements of residents.
Key Responsibilities:
1. Monitor residents' physical, emotional, social, and cognitive well-being.
2. Collaborate with the resident care coordinator to arrange necessary transfers to different care levels.
3. Keep physicians and families informed about changes in resident conditions and ensure adherence to medical instructions.
4. Assist in evaluating applicants' functional levels as needed.
5. Review all medical documentation in collaboration with the resident care coordinator.
6. Ensure completion of admission documentation prior to resident entry.
7. Assess health and functional capabilities of each resident.
8. Communicate health status updates to residents and their families.
9. Work alongside primary physicians to coordinate care and medical orders.
10. Maintain accurate nursing records for all residents while ensuring confidentiality.
11. Direct and manage resident care in the absence of the resident care coordinator.
12. Ensure adequate staffing coverage at all times.
13. Support the setup of medication assistance programs for residents as needed.
14. Supervise resident care assistants to uphold a safe medication assistance protocol.
15. Manage the proper storage of medications not retained by residents.
16. Maintain and review medication records, including new prescriptions and changes, ensuring compliance with health regulations.
17. Provide education to residents regarding medications and medical equipment.
18. Participate in on-call duties as required to address resident and program emergencies.
19. Conduct training sessions for resident care assistants as necessary.
20. Engage in quality assurance processes and audits related to nursing programs.
Qualifications:
- Valid nursing assessment skills.
- Strong collaborative approach with team members.
- Ability to interact with residents, families, and staff in a courteous manner.
- Current licensure as a licensed practical nurse.
- Minimum of three years of experience working with older adults.
- Registered nurse licensure may substitute for LPN requirements and some experience.
Physical Requirements:
The role requires occasional walking and standing, with the ability to lift up to 50 pounds without assistance. The employee must possess good vision capabilities, including close and distance vision.
Working Conditions:
The majority of duties are performed indoors, in areas where universal precautions are applicable. Employees may experience fluctuations in indoor temperatures.
Supervisory Responsibilities:
The position involves assisting in the supervision of resident care staff, ensuring adherence to established guidelines.
Safety Responsibilities:
Employees must be familiar with all safety policies and procedures, actively participating in fire drills and emergency protocols.
