We are seeking a highly skilled and compassionate Care Navigator to join our team at the Brien Center for Mental Health. As a Care Navigator, you will play a critical role in providing outreach, engagement, support, care coordination, assessment, and person-centered treatment planning for our adult clients.
Key Responsibilities- Provide Support and Outreach: Engage with clients to integrate and coordinate care, support care transitions, and deliver treatment.
- Culturally Sensitive Care: Identify cultural and linguistic needs and preferences and deliver person-centered care based on those needs.
- Barriers to Care: Continuously identify and resolve barriers to meeting goals complying with the Person-Centered Treatment Plan.
- Therapeutic Groups: Facilitate therapeutic groups, as needed, to serve individuals.
- Long-Term Support: Provide long-term support to individuals who have completed an episode of treatment with a clinician but requires on-going support.
- Treatment Planning: Collaborate with the treatment team to develop and implement a treatment plan.
- Progress Evaluation: Evaluate progress towards treatment plan goals and modify as needed.
- Referrals and Transportation: Facilitate referrals to care, and other supports to ensure that clients attend appointments, and assist with transportation as necessary.
- Community Navigation: Assist individuals in navigating the network of community-based services and information.
- Safe Transitions: Support safe transitions of care for individuals moving between care settings and services.
- Communication: Facilitate communication between individual or designated representative and medical, behavioral, and social service providers.
- Meetings and Documentation: Attend meetings as required or requested, and maintain client records, completing all required documentation in a timely and accurate manner.
- Education: BA/BS in a relevant Human Services field required.
- Experience: Experience in community-based behavioral health support programs preferred.
- Skills: Ability to work with diverse communities, support diversity, equity, and inclusion initiatives, work on an interdisciplinary team, and possess excellent oral and presentation skills.
