Resident Intake and Coordination Lead

VestraCare is seeking a Resident Intake and Coordination Lead to join our team. As a key member of our admissions team, you will be responsible for coordinating all aspects of resident intake and ensuring a seamless transition into our facility.

Your responsibilities will include:

  • Screening and recommending patients for long-term or subacute care
  • Visiting designated hospital assignments daily
  • Collaborating with nursing staff and social services to develop individualized care plans
  • Maintaining accurate records of resident information and transfer documentation

We require a Bachelor's Degree or equivalent experience in Social Work, Nursing, Therapy, or another health-related field from an accredited institution. A valid New York State Driver's License and reliable transportation are also required.

This is a Full-Time position with a competitive salary range of $26.00/hr to $34.00/hr dependent on clinical experience and licensure.
